Part One
Presenters: Lynda Brachman Cooper, LCSW Director Community Development Home Care Network, Northwell Health
Shannon Prasad, BS Program Development Manager RegionCare Nursing Agency, Northwell Health
Description:
The first portion of the program is to provide social work and healthcare professionals with current theory, resources and technology for aging in place in private home or naturally occurring retirement communities. This program will encompass speakers to address healthcare systems with case examples.
Learning Objectives:
After the completion of this workshop, participants will be able to:
- Define elements of the continuum of care and its importance to successful aging in place.
- Describe the range of home-based services that address medical, personal care and psychosocial needs.
- Describe how the same clinical scenario can result in different plans based on a variety of factors.
- Define the various kinds of home care. Identify the challenges in understanding benefits and financial resources.
- Discuss interdisciplinary communication between agencies.
- Review clinical scenarios and crisis interventions.
- Ethical considerations in planning for care including cultural awareness and social diversity.

Amy Nathan, Aging Program Coordinator for the Suffolk County Office for Aging (SCOFA)
Joseph Delgado, CEO of Suffolk Independent Living Organization
Description:
The second portion of the program will include the panel presentation by agency service providers. Panelists will inform and discuss the scope of services available to assist social work and healthcare professionals in their work. Discussion will include the availability of home energy services, in home services and technology resources to promote maintaining dignity at home.
Learning Objectives:
The goals of this workshop are to:
- Provide education to social work and healthcare professionals of the scope and the availability of community resources.
- Presentation will pertain to community-based home energy, technology, equipment, adaptive devices and support programs to promote independence at home.
- Review assessments to identify strengths and individual needs to establish a person-centered plan for aging in place.
- Discussion of cost effectiveness of community-centered care and evolving services (telehealth, telepsychology, electronic information technology).
This workshop is approved for 4.0 continuing education credit hours.

Amy Nathan is the Aging Program Coordinator for the Suffolk County Office for Aging (SCOFA). She will speak about SCOFA's mission to encourage, preserve independence and dignity for seniors. Her presentation will include descriptions of County Programs and Services available to seniors, accessing those services and programs and the NY Connects resource and referral services for seniors, caregivers, families and healthcare professionals.
Joseph Delgado is the CEO of Suffolk Independent Living Organization (SILO). He will be speaking about the mission of SILO as the voice of people with disabilities and describe the programs and services available to seniors through SILO and SILO's role in connecting seniors to services that foster, enrich and maintain their independence in the community.
Michele Bontempi is an LCSW Account Manager with CLEAResult National Grid Home Energy Affordability Team (HEAT). She will speak about the array of HEAT energy education and programs that offer no-cost energy efficiency services available to income-eligible senior homeowners and renters.
Lynda Brachman Cooper, LCSW is the Director of Community Development for Northwell Health At Home. She oversees the Joint Commission Certified Community Based Palliative Care Program, Aging in Place Initiatives and a Geriatric Care Management Program, Circle of Care.
Shannon Prasad is the Program Development, Manager for Northwell Health At Home the licensed home care agency owned by Northwell Health. She is currently a member of the Advisory Board for Touro Senior Citizen Law Program and board member for the Long Island Coalition for Aging. Her mission is to educate the community and professionals on how to keep seniors in the community safely for as long as possible..
