The Behavioral Health Social Worker/Mental Health Counselor will work directly with patients, with primary care providers and health advocates crafting, facilitating, and maintaining care plans tailored to patients’ needs. You’ll conduct care coordination and assessments to identify barriers to seeking medical and behavioral health care. You will be the primary liaison with community organizations. You will:
Consult with patients, understanding and identifying their social needs to matching them to resources
Collaborate with a team to craft personalized care plans
Provide support and guidance to an interdisciplinary care team
Conduct psychosocial evaluations (including mental status exams), translate to care plan goals, provide crisis intervention, provide short-term evidenced based therapy, as needed
Foster meaningful relationships with organizations in the community
