Workshop Description

This workshop will discuss a new consensus study from the National Academies of Sciences, Engineering, and Medicine entitled, Integrating Social Care into the Delivery of Health Care. The report, which focuses on integrating social care and medical care, highlights the role of social workers in addressing social care needs. This continuing education workshop will present the final report and recommendations of the committee, with detailed discussion on the impetus for the study; the process undertaken by social work advocates to bring it to fruition; social work best practices interventions ,and impact findings identified by the study as well as opportunities and plans for acting on the recommendations. 

 

Learning Objectives

After the completion of this webinar, participants will:

  • Describe the final report from the National Academies of Science, Engineering and Medicine entitled, Integrating Social Needs Care and the Delivery of Health Care: Moving Medicine Upstream.
  • Explain key findings and recommendations regarding social work’s role in population health initiatives, and initiatives underway to help disseminate them.
  • Illustrate how two social-work-led care models have achieved improved patient outcomes by leveraging social work core competencies, fostering collaboration between medical and social needs care providers, and focusing on fidelity and scalability.

This workshop is approved for 1.5 continuing education credit hours.

Victoria M. Rizzo, LCSW-R, PhD, Associate Professor, School of Social Welfare, University at Albany, SUNY has dedicated her career to demonstrating the value of social work in health care settings through her research, teaching, and workforce development activities. Dr. Rizzo’s research examines the impact of social work, and interprofessional interventions that include social workers, on health care utilization, health, and behavioral health outcomes for older adults coping with chronic illnesses, elder abuse, and care giving. She also examines the implications of the findings of her work for health care policy and the financing of the provision of social work services to older adults and their caregivers. Dr. Rizzo has extensive experience as a health care social worker and has conducted research with colleagues at Kaiser Permanente Health Care Systems, Rush University Medical Center in Chicago and the Jewish Association Serving the Aging (JASA) in New York City. Dr, Rizzo served as a member of the consensus report plannins committee and an external reviewer for the report that is the focus of this workshop. Dr. Rizzo has published 39 peer-reviewed articles and book chapters. She co-edited a textbook, entitled Gerontological social work and the grand challenges: Focusing on policy and practice (2019). Dr. Rizzo is a distinguished scholar and fellow of the National Academies of Practice (Social Work Academy), a Fellow of the New York Academy of Medicine, and a John A. Hartford Geriatric Social Work Faculty Scholar.
