Workshop Description

With an increasing focus on length of stay, readmission, and reimbursement, the goal of this training is to empower the voice of the social worker to identify patients whose goals of care should be changed from aggressive treatment to comfort/hospice focused measures. Data shows that many Americans prefer to die at home however, very few are granted that opportunity. Moreover, hospice has been known to save hospital systems dollars by preventing readmissions and lowering length of stay. This training will help social workers to engages providers, patients, and families in the difficult conversations related to Palliative Care and Hospice as well as transition patients and families on to these programs. 

 

Learning Objectives

After the completion of this webinar, participants will be able to:

  • Define and identify the differences between palliative care and hospice
  • Identify key points in patients’ illness in which to discuss palliative care goals and appropriate times to discuss comfort care and hospice
  • Break bad news regarding terminal illness from the provider perspective
  • Identify the signs of impending death

This workshop is approved for 1.5 continuing education credit hours.

Rebecca Freeman LMSW, MHA, ACHP-SW, is a licensed social worker who, while earning her Masters of Social Work Degree at New York University, completed the Zelda Foster End of Life/Palliative Care Focused Learning Opportunity. It was during this time that she developed a love and a passion for patients and families facing terminal illnesses and end of life. Upon completion of her MSW degree Rebecca was accepted and completed an Interdisciplinary Palliative Care Fellowship Program through the Bronx Veteran’s Affairs Medical Center. Prior to joining the Palliative Care team at Winthrop-University Hospital, she was a member of the Palliative Care team at Bellevue Hospital where she served not only as a social worker but coordinator of volunteer and bereavement services as well as on the Ethics Committee. Rebecca currently works as the Business Development Manager for Hospice of New York. With a commitment to honoring patients’ wishes especially at the end of life, Rebecca continues to advocate for patient’s to be in their desired setting during this transition period despite the complex discharge planning that is often involved. Rebecca has served as an educator to medical students, residents, attending physicians, as well as the social work staff and trainees. She is a recipient of the 2016 Social Work Hospice and Palliative Care Network Emerging Leaders Award.
